Each hospital in the Netherlands provides different follow-up care. Many hospitals have a special outpatient clinic for follow-up care, but this may also be arranged via a home care organization. Often there are regional agreements in place. You can discuss this with your treating physician. The follow-up care is often not tailored specifically to people who have suffered a stroke at a young age.
